Dear Patient:

We appreciate your selection of our office for your cosmetic needs.

Download below our Patient Registration Form, Medical History Questionnaire and Consent Form. Please complete these forms at home as completely as you are able and bring them with you to your appointment. Also, please bring a list of any medications and eye drops you are currently using, along with your eyeglasses or contact lenses. Be sure you have your insurance cards, and primary physician referrals, when required, when you arrive to our office. If consulting with Dr. Pacheco regarding eyelid procedures, please bring an old photograph of yourself, an 8 x 10 high school graduation photo would be ideal.

Applicable copayments and deductibles will be collected from you for all insurance plans with which we participate. Payment is requested at the time of the office visit for any copayments or non-insurance service. For your convenience, we accept personal checks, cash, Mastercard and Visa.
